We connect people to community groups for practical and emotional support

Social prescribing and community-based support is part of the NHS’ Long Term Plan. It’s a commitment to make personalised care business-as-usual across the health and care system.

Social prescribing enables all member practices to refer people to a link worker. Link workers give people time and focus on what matters to the person, as identified through shared decision making or personalised care and support planning. They connect people to community groups and agencies for practical and emotional support. Link workers collaborate with local partners to support community groups to be accessible and sustainable and help people to start new groups.

Eligible Criteria:

  • The patient must be over 18 years old

  • Express consent must be given to engage with the service and patient record to be accessed by a third party organisation (Gateway Family Services CIC)

  • Referrals are welcomed for patients who would benefit from non-clinical support in relation to a range of social risks. To include but not limited to:

    • Difficulty accessing other services

    • Social isolation

    • Low mental health and well-being

    • Financial hardship

    • Housing issues

    • Welfare/Benefits advice

    • Domestic abuse

    • Carer/former carer

    • Recently arrived in the UK

    • Alcohol abuse
