Enhanced Support Service for Care Homes

 
 

Our Federation is proud to offer a dedicated Enhanced Support Service for Care Homes.

All Federation members who register patients for this service will receive, on an ongoing and permanent basis, virtual support for urgent medical conditions. They will also enjoy a range of other benefits that are exclusive to this service.

The virtual support:

  • Is available 8am - 8pm 7 days per week

  • Gives advice for acute conditions

  • Responds to acute enquiries

  • Manages urgent prescriptions

  • Offers telephone and video consultations

Along with this valuable virtual support, our Enhanced Support Service for Care Homes also offers members who have registered patients:

  • Chronic disease management

  • Regular ward rounds

  • Drug monitoring, medicines reviews and repeat prescriptions

  • Help communicating with relatives

  • Death certificates

  • Clinical referrals

  • Full range of General Medical Services

  • Vaccinations

This is an integrated service with Birmingham Community Healthcare NHS Foundation Trust and Birmingham and Solihull Mental Health NHS Foundation Trust. It provides a MDT approach, which is led by GPs and includes:

  • Advanced Nurse Practitioners

  • Clinical Pharmacists

  • Physiotherapists

  • Occupational Therapists

  • Speech and Language Therapists
