Helping vulnerable patients manage their mobility

The SDSmyhealthcare Frailty MDT Community Clinic is a virtual review. Chaired by a GP, it is attended by a number of specialists including some from BCHC. The clinic provides advice and guidance to establish and manage care plans for referred patients, based on the pooled knowledge of all available MDT members.

The MDT Reviews are currently running monthly as part of a pilot scheme. Our vision is to expand it further across the Federation.

 

Who can use the clinics?

The Community Clinics are available to all Federation practices.
